Paypal will show a $0 item for the creating of the recurring package with a note “created”, and the $159 should be a separate transaction on the same day with a note “completed”.
I did a test and cannot find a problem …

Cities will never load, it is a free text field.
When selecting Canada, the provinces will load.
But you have to wait until the page is fully loaded, and that depends on your server.I am not sure what you mean with “customer is unable to select the city”.
City is a free text field.I had no trouble changing country to Canada and selecting a different province and add a city.
It went OK to the payment gateways too.I did turn off one of your caching plugins, you should only use one.
Please turn off the security plugin if we need to have a look.
Sorry, but we do not support customizations.
Recurring packages need to be activated by a user in the chosen payment gateway.

Thanks1. Delete all your test invoices.
Then go to Invoicing > Settings > Misc tab >
Sequential Invoice Numbers > Yes
Sequential Starting Number > 12. Payment can only be successful if an invoice was generated as part of the listing submission process.
Once a payment was successful, you should not modify any invoice generated. At any rate, the invoice number cannot be changed after successful listing submission, regardless of payment.Thanks
